Agency Adoption Requirements
Pursuant to O.R.C. §3107.02(B) An adult may be adopted under any of the following conditions:
1. If the adult is totally and permanently disabled.
2. If the adult is determined to be a mentally retarded person as defined in §5123.01 of the Revised Code.
3. If the adult had established a child-foster caregiver or child step-parent relationship with the petitioner(s) as a minor, and the adult consents to the adoption.
§ Filing Fee: $142.00
§ Petition for Adoption
One typed and signed copy. Name appearing on petition will be the name appearing on the new birth certificate. Include first, middle and last name of petitioner(s) and adoptee as they are to appear on the new documents. Due at the time of initial filing.
§ Certified copy of adoptee’s birth certificate
Due at the time of initial filing.
§ Petitioner’s Preliminary Account
Due at the time of initial filing. Final Petitioner’s Account due 10 days prior to hearing date.
§ HEA 7257 Certificate of Adoption Form
All items must be completed on this form. Due at the time of initial filing.
§ Consent to Adoption executed by the adoptee.
