§ Petition for Adoption
One typed and signed copy. Name appearing on petition will be the name appearing on the new birth certificate. Include first, middle and last name of petitioner(s) and adoptee as they are to appear on the new documents. Due at the time of initial filing.
§ Certified copy of adoptee’s birth certificate
Due at the time of initial filing.
§ Judgment Entry Setting Hearing and Ordering Notice
§ Petitioner’s Preliminary Account
Due at the time of initial filing. Final Petitioner’s Account due 10 days prior to hearing date.
§ HEA 7257 Certificate of Adoption Form
Due at the time of initial filing.
§ Permanent Surrenders of Birth Parents and accompanying Affidavits if a minor parent. Must have the original signatures.
Due at the time of initial filing.
§ ODHS 1693 Ohio Law and Adoption Materials with original signatures.
§ Certified copy of Juvenile Court Order terminating parental rights due to abuse, neglect and dependency.
The entry must give permanent custody of the adoptee to the agency. If the court order filed is a Magistrate’s Decision you must also file the Judgment Entry Adopting Magistrate’s Decision.
§ Criminal Record check
Pursuant to O.R.C. 2151.86
Must be dated within one (l) year
of the filing of the Petition.
§ Consent to Adoption of the Agency or Juvenile Court (O.R.C. 3107.06) (whichever applies)
Must have consent filed prior to
finalization.
§ Consent to Adoption by the Adoptee if twelve (12) years or older
This consent must be executed in
Court before the Judge or
Magistrate.
§ ODHS 1699 Pre-finalization Assessment Report
Due twenty (20) days prior to final
hearing.
§ ODHS 1698 Home Study Report
Due ten (10) days prior to hearing
date.
§ ODHS 1616 Social and Medical History of Birth Parents
Must identify adoptee by the adoptive name. This form is comprised of non-identifying information.
§ Proof of marriage of petitioner(s) if married in a county other than Mahoning County
§ Certified copy of all divorce decrees of both adopting parents, if applicable.
§ Certified copy of death certificate of any previous spouse of the petitioner.
